If you are the parent of a student with an intellectual disability who currently is enrolled in a Tennessee high school, please participate in this anonymous survey.

The Survey on Post-Secondary Education Options for Students with Intellectual
Disabilities collects information about:

  • You and your son or daughter withan intellectual disability
  • Transition plans for your son or daughter after high school
  • Your opinions about the options available to your son or daughter
  • If your son or daughter is 18 yearsor older, his/her opinions about the options available after high school

Surveys should be completed by 8/31/08
